As the first step in preparing to take doctoral qualifying exams, you need to submit the Request to Take Doctoral Written Qualifying Examinations form. It is available online here and on paper in the Music Graduate Office.
Please fill out all of the information requested in the form below, then submit it by clicking the submission button at the bottom. Prof. Isaacson, Director of Graduate Studies, will reply by e-mail with information about your eligibility to schedule each of your written qualifying exams; this can take up to a week. Once you are eligible to schedule an exam, see the written exam calendar to reserve a date.
If you have questions, please contact the Music Graduate office at musgrad@indiana.edu, 812-855-1738.
Please note that putting a date in the field above does not actually reserve an exam date. It is for general information about when you plan to start exams.
Please note that the information requested below is to report things that you have already formally declared. If you have not yet formally declared your minor fields, please see here for instructions. If you have not yet formally declared your Major Field Advisory Committee or your Minor Field Representative(s), see here for instructions.

Anything you write on will be included with your exam when it is sent to be evaluated.
