Program Planning Sheet - Graduate Certificate Students
We will begin accepting Program Planning Sheets for Spring 2023 on Monday, October 17.
Please note: After you submit your program planning sheet you will receive an immediate email confirming that your submission was successful, followed by an email from the Music Graduate Office informing you that you are eligible to register after your plans are approved. You will not be able to register until after you receive this second reply. Replies will usually go out within 24 hours (not including weekends) of program planning sheet submission, but sometimes take longer. In busy periods, it is very likely to take up to 5 weekdays to get a reply, but be assured that all program planning sheets are processed in the order in which they arrive. If you do not receive a reply after 5 days, please contact us.
As the first step in registering for classes each semester, you need to submit the Jacobs School of Music's Program Planning Sheet. Click here for more information about registration, enrollment policies, and schedule adjustment.
Please fill out all of the information requested in the form below, then submit it by clicking the "continue" button at the bottom of the page, then "submit" on the next page. You will get a copy of your program planning sheet by email right away. The Music Graduate Office will send an additional e-mail once you are eligible to register; please read that reply carefully for registration instructions and for reminders about requirements.
Course information will be listed in the Schedule of Classes.
Certificate students must enroll in the number of credit hours per semester required for their program.
