Program Planning Sheet - Diploma and Visiting Students
Program Planning Sheets for Summer and Spring 2025 will be accepted starting Monday, October 21.
Please note: After you submit your program planning sheet you will receive an immediate confirmation of your submission, followed by a later email from the Music Graduate Office informing you that you are eligible to register after your plans are approved. You will not be able to register until after you receive this second reply. Replies will go out as soon as possible after submission, but sometimes take a couple days. In busy times, it is very likely to take up to 5 weekdays to get a reply, but be assured that all program planning sheets are processed in the order in which they arrive. If you do not get a reply after 5 days, please contact us.
As the first step in registering for classes each semester, you need to submit the Jacobs School of Music's Program Planning Sheet. Click here for more information about registration, enrollment policies, and schedule adjustment.
Please fill out all of the information requested in the form below, then submit it by clicking the "continue" button at the bottom of the page, then "submit" on the next page. You will get a copy of your program planning sheet by email right away. The Music Graduate Office will send an additional e-mail once you are eligible to register; please read that reply carefully for registration instructions and for reminders about requirements.
Course information will be listed in the Schedule of Classes.
Diploma and Visiting students must enroll in at least 10 credits hours for fall and spring semesters. There is never an option for diploma or visiting students to register in less than 10 credits in the fall or spring semesters in which they choose to enroll.

We are asking you to define the "Requirement Category" of the classes you will be taking. Some of those categories are already filled in on the program planning sheet for you (things like Performance Study [lessons], Masterclass, and Ensemble). For categories that are not already filled in, you will need to choose a course category from the drop-down menu. For example, if you are going to take MUS-G 560 Graduate Choral Conducting towards the music course requirement, you will choose the "diploma music course" category from the drop-down menu for the Requirement Category.
Visiting students can use the "optional class" category for everything other than Performance Study and Ensemble (since there are no specific program requirements).
If you aren't sure about which category a course fits into, then put your best guess and then make a note in the notes field at the bottom of the form to let us know what you weren't sure about.
